Add Questionnaire

Modules > COI > Questionnaires > Add Questionnaire

Add a Questionnaire when you want to start the COI process.

To add a COI Questionnaire, complete the following steps:

  1. Go to Modules > COI > Questionnaires.
  2. Click the Add Questionnaire link.

    Note: You can also click the Add Questionnaire hyperlink found directly on the COI Overview page.

    The Add - Questionnaire page opens.

  3. Complete the following fields as needed:
    Field NameRequired?Description
    questionnaire name Required Enter the name of the questionnaire. This is the name that will be seen by users on eWeb.
    eWeb description Required Enter a short description of this questionnaire in this field. This description will be seen by users on eWeb.
    purpose Optional Enter a short description of the intentions of this questionnaire and what it will be used for.
    from date Optional Enter the start date on which this questionnaire will be available on eWeb.
    through date Optional Enter the end date on which this questionnaire will no longer be available on eWeb.
  4. Click the Save button to save your changes and return to the Questionnaire Profile page.Questionnaire Profile page.
